Insurance certificate must be on file prior to rental.
Drivers charge is portal to portal based on a 10 hr. day
Additional equipment not part of the package is charged as loaded.
Truck rental price does not included driver's wages, fuel or mileage
A fuel sur-charge of 60 cents a mile will be added to the final invoice.
Rental agreement must be signed prior to job with terms and conditions.
Customer is responsible for tolls, permits, parking fees and any parking violations.
Driver must be an Andrus Lighting and Grip approved grip driver or gaffer/electrician.
Payment shall be upon completion of job, unless other arrangements have been specified.
